Thank you for your interest in enrolling at Bridgetown Primary School. Parents seeking to enrol their child for Kindergarten will need to complete the following Enrolment documents. These are available to download from the Website via the link or by requesting copies from the Bridgetown Primary School office.
We also require:
- Your child's birth certificate (telephone the Registry of Births, Deaths and Marriages on 1300 305 021 if you do not have one).
- Your child's Immunisation History Statement (no older than 2 months old), (for instructions on how to obtain this statement visit the Department of Health).
- Proof of your current address (such as a recent electricity or water statement).
- Copies of Family Court orders for confirmation of proof of name or custody arrangements.
Please ensure all contact details are correct. It is vital that in an emergency, we are able to reach parents immediately. Should parents be unavailable, the next emergency contact supplied will be called.
These documents can be completed and emailed back and we will print them out, or you can bring them to the office. Once we have received the completed forms, we will be in touch to confirm your enrolment.
